The fine print:

1. Prescription refills, home visits or same day consults is at your Physician’s discretion.
2. Providers will be available by phone, text and email. In the event that your call reaches voicemail, your call will be returned with in 48 hours
3. Prescription refills are within 72 hours.
4. **Health For Your Whole Life, LLC reserves the right to limit the number of text messages, emails and phone calls, and terminate your service at any time, without refund.
5. Direct access is reserved for Concierge members only.
6. Direct access for non-members will be charged at the billable rate.
7. After hours, house calls, consultations, written requests, prescription refills and holiday requests for non-members incur additional charges as follows:
   • New Patient consult: $450
   • 30 min Follow-up consult: $150
   • 60 min Follow-up consult: $300
   • House call surcharge: $200 up to 15 mile radius, $300 >15 mi to 30 miles, $500 > 30 mile radius
   • Holiday consult surcharge: $200
   • Medical Letters, Narrative Reports: $150 (minimum)
   • Lab work requisitions (not accompanied by a visit): $75
   • Prescription Refills: $75
8. All Naturopathic Assurance Concierge Plans are billed require a 12-month commitment and are billed monthly. There are no refunds, and charges are not pro-rated.  All plans are available on a limited basis and are subject to change. *Additional 3.5% fee for all credit card charges
9. House calls are limited to the Phoenix metro area. Additional monthly charges may apply for Concierge patients outside the Phoenix Metro Area.

*Please note that all credit card payments include a 3.5% processing fee. All lab costs are the responsibility of the patient except where noted. Direct Doctor phone calls, texts or emails requiring extensive Physician research and/or time will be billed at the corresponding visit rate. All new prescriptions or prescription refills may require a consult and are at physician discretion. No additional discounts for monthly IV therapy beyond the $150/month credit. Dr. Tondora’s office/Health For Your Whole Life does not accept insurance and is not a Medicare provider.  An insurance policy is a contract between you and your insurance company.  The patient is ALWAYS responsible for payment of all charges incurred regardless of any insurance or third party payment arrangements.  Medical health savings accounts are an acceptable form of payment. The natural medicines prescribed by Dr. Tondora and other HFYWL Providers may be purchased online, at the office, or at a pharmacy of your choice.
